The ‘Whatever It Takes’ program (WIT) helps the service system respond to the needs of infant, children and youth aged 0 to 18, with complex clinical profiles and complicated service needs.
WIT provides short term intensive clinical case management services to the entire service delivery system – when the system is experiencing difficulties finding, or providing, appropriate services for these children, youth and their families. WIT is based on developing a collaborative approach with service providers and families, that aims to provide a continuum of services and supports from various sectors such as Health, Education, Justice, Child Welfare and Children’s Mental Health.
Strides Toronto’s WIT program serves the East Toronto (Scarborough) and South Toronto (Victoria Park Avenue to Keele Street, South of Eglinton).
For WIT services in North and West Toronto please contact Lumenus at (416-222-1153) or wit@lumenus.ca.
Eligibility
Clients must have complex clinical profiles (mental health concerns, multiple diagnoses, rare diagnoses, medical challenges, developmental concerns etc.) and have experienced complicated service situations (multiple service providers across sectors i.e. children’s mental health, education, hospitals, child welfare etc.).
Referral Information
- Service providers with knowledge of the youth and families needs can make referrals. The expectation is that service providers have exhausted the system and require a collaborative and individualized clinical case management approach.
- Service providers should call intake at Strides Toronto at 416 438-3697 x 19055 and ask to make a referral to WIT. Once contact is made a referral form will be forwarded to the service provider.
