Strides Youth in Transition (YIT) Worker Program Referral Form

The Strides Youth In Transition (YIT) Worker Program supports youth 16-24 in their transition out of the child welfare system into adulthood. YIT workers help youth to identify, access, and navigate adult service systems relevant to their specific needs and goals.

To be eligible, youth must be between 16 – 24 years, and receiving one of the following:

• Extended Society Care (previously referred to as Crown Ward)
• Ready Set Go (RSG) Program
• Voluntary Youth Services Agreement (VYSA)

The goal of our program is to:  

•  Assist youth transition from child welfare into adulthood.
• Connect youth to information, options, life skills and resources.
• Create stability and connections to family, roots, community and culture.
• Work with youth to develop and achieve age & stage appropriate goals.

Once your referral form has been received, staff will make contact with the young person within 5 business days. If we require more information about the young person, we will place a call to you first before initiating contact.  If you have any additional questions, please contact us at yit@stridestoronto.ca.
